Observation

During onsite inspection, Licensor observed the stairs leading to the second floor of family home and stairs leading to the outdoor play area accessible and not equipped with a gate. (4)(f) During onsite inspection, Licensor observed six outlets accessible in proposed license space not equipped with a tamper resistant outlet or automatic shutter. (5)(a) During onsite inspection, Licensor observed an outlet 16 inches from sink in the proposed licensed bathroom not equipped with a GFCI outlet or blank plate. (5)(b) During onsite inspection, Licensor observed an extension cord replacing a direct outlet (TV, wireless router, and cable box plugged in) in the proposed licensed living room.
